A North Carolina law requires all students new to North Carolina Schools, or entering Kindergarten for the first time to have a PHYSICAL EXAM. The physical must occur within the last 12 months and be signed by a physician. You may share the Health Assessment form below with your child's doctor.

- At the end of the online process, schedule an appointment to come see your new school! Please bring these documents needed to enroll your child in school:
- Proof of Residency: Students are assigned to schools based on where their parent or legal custodian lives. Parent/legal custodian must show proof of residency with one of the following:
- Mortgage/Lease/Rental Contract
- Utility Bill (water, electric, gas, cable, land-line phone)
- Driver’s License of Parent/custodial guardian (Picture ID is acceptable)
- Certified Birth Certificate (NCGS 115C-364 states a certified copy is required for enrollment)
- Immunizations (NCGS 130A-155 allows 30 days for immunizations to become a part of the cumulative record. After 30 days the student will be suspended or required to begin the vaccine series.)

The Back to School Forms for 2022-2023 school year are now CLOSED for those that were enrolled in Davie County Schools last year.

I can’t remember my username or password for PowerSchool Parent Portal.
Use the “Forgot Username or Password” link on the Sign In page. Please contact your child's school if you need further assistance logging in to Parent Portal.
What if I do not have a PowerSchool Parent Portal Account?
Complete the Parent Portal Access Request form and upload a Photo ID. If you are unable to complete the online request, contact your child's school for more information on how to create a Parent Portal account and connect your child.
In County Transfers
For those that have moved within Davie County or been granted reassignment for the first time
Your child is only allowed to transfer from one school district to another within the county if you have moved and your new address is located inside the different school district or if you have applied for reassignment. If you need to find your new school district, please click here for school eligibility.
If you have moved, please call your new school for an appointment. All school contact information is on this page. Bring the following documents to your appointment:
