A North Carolina law requires all students new to North Carolina Schools, or entering Kindergarten for the first time to have a PHYSICAL EXAM. The physical must occur within the last 12 months and be signed by a physician. You may share the Health Assessment form below with your child's doctor.

For those that are not currently enrolled in Davie County Schools!
We are so excited that you are joining our schools!
Step One:
Gather documents to upload to the online enrollment form.
- Proof of Residency: Students are assigned to schools based on where their parent or legal custodian lives. Parent/legal custodian must show proof of residency with one of the following:
- Mortgage/Lease/Rental Contract
- Utility Bill (water, electric, gas, cable, land-line phone)
- Driver’s License of Parent/custodial guardian (Picture ID is acceptable)
- Certified Birth Certificate (NCGS 115C-364 states a certified copy is required for enrollment)
- Immunizations (NCGS 130A-155 allows 30 days for immunizations to become a part of the cumulative record. After 30 days the student will be suspended or required to begin the vaccine series.)
- Safe Schools Enrollment Declaration Under North Carolina Law, General Statute 115C -366(a4), enrollment to a school requires the student's parent, guardian or legal custodian to provide a statement, made under oath before a qualified official. The statement indicates whether the student is under suspension or expulsion from any private or public school in this or any other state or has been convicted of a felony in this or any other state.
- Health Assessment Form: If this is your child's first time in North Carolina Schools, our state has a law requiring all students new to North Carolina Schools, or entering Kindergarten for the first time to have a PHYSICAL EXAM. Step Four:
At the end of the online process, schedule an appointment to come see your new school! If you were unable to upload the documents listed in Step One when completing your online forms, bring them to your school or email copies of them to studentinfo@davie.k12.nc.us.

Annual "Back to School" Updates - 8/1/2024 - 11/1/2024
Davie County Schools has transitioned from PowerSchool to Infinite Campus. Parents will receive an email from studentinfo@davie.k12.nc.us with instructions for how to create your new account and complete your annual updates. It is imperative to review and update your information to ensure is converted correctly!
Parent PortalLog in to your Parent Portal account to complete your annual "back to school" forms.
I can’t remember my username or password for the Infinite Campus Parent Portal.
Please email studentinfo@davie.k12.nc.us or call the Student Information office at 336-751-5921 to have your password reset.
What if I do not have an Infinite Campus Parent Portal Account?
Complete the Parent Portal Access Request form and upload a Photo ID. If you are unable to complete the online request, contact your child's school for more information on how to create a Parent Portal account and connect your child.
In County Transfers
For those that have moved within Davie County or been granted reassignment for the first time
Your child is only allowed to transfer from one school district to another within the county if you have moved and your new address is located inside the different school district or if you have applied for reassignment. If you need to find your new school district, please click here for school eligibility.
If you have moved, please call your new school for an appointment. All school contact information is on this page. Bring the following documents to your appointment:

If you have not provided one to your previous school, a Certified Copy of Birth Certificate (NCGS 115C-364 states a certified copy is required for enrollment)
If you already have an account from a previous enrollment you can use the same login information. If you do not have internet access at home, then you can visit the public library or your school to access the websites.
